Other charges include child endangerment while allegedly operating under the influence.

A collision on Forest Avenue by theon Wednesday led to the arrest of a Swampscott woman on charges of OUI, second offense, and child endangerment for operating under the influence of alcohol as well as a marked lane violation.

Lenore Lynch, 44, of 1107 Humphrey St., was arraigned Thursday in Lynn District Court and released on $5,000 bail. Her pretrial hearing is scheduled for Sept. 16.

Wednesday about 2:30 p.m., the driver's pick-up truck collided with another vehicle, police said. Witnesses told police that the pick-up had crossed the road's yellow lines and hit another vehicle head-on, the report states.

Swampscott firefighters responded to the scene but there were no injuries, said Deputy Fire Chief James Potts.

The police officer who responded to the accident stated in his report that he smelled alcohol when talking to Lenore and noticed that her eyes were bloodshot.

He asked her if she had been drinking and she responded that she had been "smashed" the night before from drinking a liter of vodka, the police report states.

The officer administered field sobriety tests including a roadside breath test. The driver recorded a 0.155 on the breath test, almost twice the legal limit, the police report states.

Later, at the police station, she refused to take a breath test.

She was charged with OUI second offense and child endangerment because her daughter had been riding with her, police said.

The driver was bound for the middle school to pick up another child of hers, the report states.
