Back to School Health Information from Swampscott School Nurses

Reminder to parents and guardians: Physical Exam and Immunization Reports for students in grades 4,7,and 10 are due the First Day of School August 28.   Incoming kindergarten students' health documents are due by September 3.
 If your child requires medication in school, please provide your nurse with written permission from doctor and parent.  Medications must be in original container and delivered by and adult.  For more information see your child's school webpage with link to "Health Services".   Please contact your school nurse if any health concerns.    Laurie O'Brien RN
